Job Description

• Position: Registered Nurse (RN)
• Experience Required: Must have Telemetry experience.

Required Credentials

1. BLS Certification (AHA or ARC only) - Required for onboarding.
2. Nursys License - Must be nursing only, within 30 days of start.
3. ACLS Certification (AHA only) - Optional for onboarding.
4. License/Certificate Verification - Primary source verification required within 30 days of start.
5. FMOLHS Skills Checklist - Must be unit specific and updated annually.
6. FMOLHS Competency Exam - Passing score of 80% or higher required.
7. FMOLHS Core Competencies - Must be current within a year prior to start.
8. MMR Immunization - Proof of 2 immunizations or positive titer required.
9. Fit Test - Must be completed within 1 year prior to start.
10. Hepatitis B Immunization - Proof of 3 immunizations or positive titer required.
11. Influenza Vaccine - Required during flu season; exemptions accepted with documentation.
12. TB Test - Proof of negative blood test or skin test required within one year prior to start.
13. Health Statement - Signed and dated within 1 year of start stating ability to work without restrictions.
14. Varicella Immunization - Proof of 2 immunizations or positive titer required.
15. Tdap Immunization - Proof of vaccination within past 10 years or declination accepted.
16. Annual TB Questionnaire - Required within 6 months of start.
17. FMOLHS Resume - Required for submission.
18. Professional References - 2 references required; 1 from a Charge Nurse or higher and 1 from a supervisor.
19. Criminal Background Check - Must include county criminal searches and education verification.
20. Drug Screen - Negative 10 Panel required within 30 days prior to start.
21. FMOLHS Badge Photo - Must be a forward-facing, well-lit, non-blurry photo with a neutral background.
22. FMOLHS Contract Annual Education - Required for onboarding.
23. FMOLHS Eye Exam & Color Vision - Must be completed within 1 year prior to start.
