Job Description

  • Candidates must have worked for a Providence-affiliated facility as permanent staff and must be separated from the health system for a minimum of 3 months to qualify for a travel contract.
  • Open to:
    • MSW Care Manager (must have either an Associate clinical license or LCISW)
    • RN Care Manager
  • Qualifications:
    • Master's in Social Work
    • Washington-issued post-MSW license (e.g., licensed social work associate independent clinical)
    • Case management and discharge experience required
    • Utilization management experience is helpful but not required
    • Average Daily Census: 18-20

Required Skills

General Skills

  • Experience in acute hospital settings
  • Knowledge of community programs and services
  • Familiarity with APS (Adult Protective Services)

Case Management/Assessment

  • Admission criteria
  • Initial evaluations
  • Care coordination and discharge planning
  • Utilization of InterQual Criteria and Milliman Guidelines

Care Coordination

  • Documenting information to facilitate care coordination
  • Participating in care conference activities
  • Referrals to rehabilitation and long-term care facilities

Documentation/Communication

  • Documentation according to the plan of care
  • Understanding of coverage criteria

Major Disease Processes

  • Required: Dementia, Stroke
  • Preferred: Cancer, CHF, COPD, Diabetes, HIV/AIDS, Neurological Disorders, Psychiatric Disorders

Regulatory Knowledge

  • Familiarity with CMS, CPT coding, HIPAA, ICD 10 coding, and OSHA regulations

Charting Systems

  • Experience with Epic and Cerner

Certifications Required

  • BCLS (Basic Cardiac Life Support)
  • ACLS (Advanced Cardiac Life Support)
  • PALS (Pediatric Advanced Life Support)
  • NRP (Neonatal Resuscitation Procedure)
  • Other certifications as applicable

Additional Information

  • Floating Policy: Travelers may be required to float to another Providence facility within a reasonable distance.
  • Background Check: Required within 45 days of start, covering all locations lived/worked within the past 7 years.
  • Health Documents: Various health documents required, including TB tests, vaccinations, and physical exams.
