Required Credentials and Qualifications
Licenses and Certifications
- License/Certificate Verification:
- Primary Source Verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are NOT accepted.
- ARRT Certification:
- Required for onboarding.
Health and Safety Requirements
- COVID-19 Vaccine:
- Proof of vaccine series or completion of medical/religious exemption form preferred.
- MMR:
- Positive MMR titers from within the last 15 years required.
- Fit Test:
- Must be completed within 1 year of start and updated annually.
- Hepatitis B:
- Proof of full series or at least two vaccines required, or signed declination.
- Varicella:
- Positive titers from within the last 15 years required.
- Tdap:
- Proof of vaccination within the past 10 years required.
- Influenza Vaccine:
- Proof of vaccination from October to March or approved exemptions required.
- 2-Step PPD or IGRA (Quantiferon/Tspot):
- Negative reads within the last 6 months required.
Background Checks
- UHS Criminal Background Check Results:
- Must be completed within 6 months prior to start.
- UHS OIG Search:
- To be completed within 6 months prior to start.
- UHS SAM/EPLS/GSA/OFAC:
- Supporting documentation required to be completed within 6 months prior to start.
Additional Requirements
- Physical/Health Statement:
- Completed within one year of initial start date, stating the traveler is able to work without restrictions.
- UHS Education Verification:
- Highest level of education verified; attestations not allowed.
- UHS Employment Verification:
- Must provide verification of employment within the last 5 years.
- UHS Skills Checklist:
- Skill-specific checklist required to be completed within 1 year prior to start.
- UHS Competency Exam:
- Must score at least 80% on relevant department exams within 1 year prior to start.
- UHS Core Competencies:
- 80% passing score required, completed within 1 year prior to start.

This OR team takes care of their equipment, allows for AM and lunch breaks, and is generally welcoming.
Cons
I experienced several unsafe surgical situations during this contract. Surgeons are not held accountable for bad behavior or bad decision-making, and surgeons tended not to take constructive criticism well. Two surgeons in particular routinely made staff cry and teams had to be switched out of his room. The filters on sterile trays were never checked except by travelers. Inefficiencies everywhere, often cases were not on-time.
