Job Description

• Candidates must ensure that the prescreen is filled out and attached upon submission.
• Required documents include:

• Candidates will be declined if these items are not completed and attached.
• Accepting new graduates or Clinical Fellowship Year (CFY) candidates.
• Skilled Nursing Facility (SNF) experience is preferred.
• Experience with Rehab Optima is preferred.
• A completion bonus of $1,000 is offered.

• County Criminal Background Check (7 years for all counties lived and worked)
• Federal Criminal Background Check (7 years or more)
• Multi-State Criminal Search (7 years or more)
• Nationwide Sex Offender Search (7 years or more)
• HHS/OIG Search
• GSA Search
• Social Security Number Trace
• One Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray
• SAM Search
• Texas DADS
• CPR (for fundamental facilities)
• Negative Drug Screen Results
