Skills Checklist - Unit specific, within one year prior to start date; renewed annually
Competency Exam - Passing score of 80% within year of start
Core Competencies - HIPAA, Patient Bill of Rights, Safety Orientation, etc., renewed annually
CPI - As required per specialty/Unit & Facility
MMR Immunizations - Proof required, positive titer or Declination accepted
Fit Test - Completeness and type specification
Hepatitis B - Proof of immunization or Declination accepted
Influenza Vaccine - Proof required, Medical/Religious exemption with signature
TB Test - Proof of negative Blood test or Skin test
Health Statement - Signed and dated within 1 year of start
Varicella - Proof of immunization or Declination accepted
Tdap - Proof of vaccination within past 10 years or Declination
Annual TB Questionnaire - Required within 6 months of start
Criminal Background Check Results - Completeness of checks
Social Security Trace - If not uploaded with background, to be complete within 6 months of start
OIG Search - Verification required within 30 days of start
EPLS - Included in background or run within 30 days of start
National Sex Offender - Verification required within 30 days of start
Employment Verification - Past 5 years
