Job Description

• Position: ICU RN
• Requirements:

  • ACLS certification required
  • Minimum of one year of critical care experience

Required Credentials

1. Certifications:

   • BLS (American Heart Association or Red Cross)
   • ACLS (American Heart Association or Red Cross)
   • Prime Basic Pharmacology Exam (Score 80% or higher)
   • Prime Basic Rhythm Interpretation Screening (Score 80% or higher)
   • Prime Specialty Exam (Score 80% or higher)

2. Licenses:

   • Nursys License Verification
   • License/Certificate Verification (Primary Source Verification)

3. Personal Health History:

   • COVID-19 Vaccine (2 doses and booster required or declination completed)
   • MMR (Proof of 2 Vaccines or Positive Titer showing immunity)
   • Fit Test (Annual)
   • Hepatitis B (Declinations not accepted)
   • Influenza Vaccine (Required October 1 - May 31; declinations not accepted)
   • TB Test
   • Varicella (Vaccination/titer showing immunity)
   • Tdap (Declinations not accepted)
   • Physical/Health Statement (Annually)
   • Prime Drug Screening (minimum 10-panel test)
   • Prime Color-blind test (Annually)
   • Positive PPD - Negative Chest X-Ray (Optional)
   • Driver's License/State ID or Passport

4. Professional Requirements:

   • National Criminal Background Check (within one year of start date)
   • Prime Agency Attestation
   • Prime SAM/EPLS
   • Prime Form I-9 (Employment eligibility verification)
   • Prime Resume
   • Prime References (2 required)
