Job Description

Role: Home Health Physical Therapist (PT)
Shift: 8-hour days (Note: 4x10s or 3x12s are not accommodated)
Patient Ratios: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
Experience Required: 1 year of skilled home health experience
First-Timers Accepted: Yes
Weekend Requirement: No
Certifications Required: BCLS
On-Call Requirement: No
Floating Requirement: No
Local Candidates Accepted: Yes (within 50-75 mile radius)
Pending License Accepted: Yes
Guaranteed Hours: Facility can call off up to one shift per pay period

Special Requirements

Transportation: Traveler is expected to have their own transportation.
Client Grouping: The system minimizes drive and windshield time, with occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
Oasis Experience: Required; please indicate if the traveler does not have Oasis experience.

Physical Exam: Required within 1 year of start, signed by a qualified healthcare provider.

- COVID-19 vaccination or declination required if onsite.
Drug Testing: 10-panel drug screen within 90 days of start.

Background Check

Requirements: Background check covering all locations lived/worked within the past 7 years, updated annually.

References

Required: Minimum of 2 references covering 1 year of employment total within the last three years, from a supervisor.
