Required Credentials and Certifications

1. BLS (Basic Life Support)

   • AHA or ARC Only
   • Required for onboarding

2. ACLS (Advanced Cardiovascular Life Support)

   • AHA or ARC Only
   • Required for onboarding

3. COVID-19 Vaccine

   • Proof of vaccine and 1 booster or exemptions must be submitted for approval.
   • Optional for onboarding.

4. MMR (Measles, Mumps, Rubella)

   • Proof of 2 immunizations, positive IGG titer, or booster if negative titer.
   • Required for onboarding.

5. Hepatitis B

   • Proof of 3 immunizations and positive antibody titer, or booster if negative titer.
   • Required for onboarding.

6. Influenza Vaccine

   • Required from October - March (unless otherwise indicated by CDPH).
   • Medical/Religious exemptions must be approved.
   • Required for onboarding.

7. TB Test

   • Must be within 30 days of start. Negative results required.
   • Required for onboarding.

8. Health Statement

   • Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
   • Required for onboarding.

9. Driver's License/State ID

   • Required for onboarding.

10. Varicella

    • Proof of 2 immunizations, positive titer, or declination accepted.
    • Required for onboarding.

11. Tdap (Tetanus, Diphtheria, Pertussis)

    • Proof of vaccination within past 10 years or declination accepted.
    • Required for onboarding.

12. Covid Test

    • Required on start date for clinical staff.
    • Optional for onboarding.

Background Checks and Verifications

• Criminal Background Check Results

  • Must be within 30 days prior to start.
  • Required for onboarding.

• License/Certificate Verification

  • Primary Source Verification of professional license, Nursys, and/or certification.
  • Required for onboarding.

• Education Verification

  • A diploma, degree, or transcripts is acceptable.
  • Required for onboarding.

Additional Requirements

• Professional References

  • 2 Professional References, with at least 1 being supervisory.
  • Required for submission.

• Drug Screen

  • Must be within 30 days of start, 10 panel standard test.
  • Required for onboarding.

• Skills Checklist

  • Clinical only: Skill specific required to be completed within 1 year of start.
  • Required for submission.

• Acknowledgment of Receipt for Policy Packet

  • Must be signed and dated by candidate.
  • Required for onboarding.

• Badge Photo

  • Neutral colored background, forward facing, no facial jewelry.
