Other Certifications/Specialty certifications requirements: PRI & SCREEN certifications (can be taken online through the School of Albany- typically take a couple hours for each certification; Cost approx. $300/certification)

Expected to have knowledge of discharge planning and making referrals.

EMR: Allscripts

Role: Will typically be working on a MS or Tele unit and will split the floor with a Social Worker.

Must be able to function independently on the unit and work collaboratively with an interdisciplinary team and patients for discharge planning.

Expected to round on the unit with physicians/team.

Need to have a positive attitude, be able to critically think, and good time management skills.

Prefer prior critical care experience.

Breaks are per facility policy.

Management is supportive of taking a full lunch break.

Weekend Requirements: Occasionally (rare- with manager approval).

Holiday Requirements: Occasionally (rare- with manager approval).

Pilot Floors will have 1 CM & 2 SWs; Caseload: 12-17 patients.

Need to be able to handle at least 20 patients.

Typically placed on Med-Surg or Tele Floors following Internal Med and Family Med.

Do you require InterQual criteria or Milliman experience: Prefer some knowledge of CarePort, MCG, and/or InterQual.

Prefer some Utilization Review experience.

Common types of patients & job duties: Discharge to Home, Community, or SNF.

Obtaining housing, Shelter Packets for Homelessness, Hemodialysis packets, KCI referrals for wound vacs, and more.
