Job Description

The Registered Nurse (RN) in Home Health is responsible for coordinating and directing home nursing care according to physician orders and agency policies. Key responsibilities include:

• Application of the nursing process in patient care.
• Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
• Conducting home visits and attending periodic office meetings.

Patient Care

• Average Number of Patients per Day: 5-8
• Types of Cases: Start of Care, Wound Care, Follow-up Visits, etc.

Experience Requirements

• Years of Experience Required: Minimum of 1 full year of Adult Home Health Experience or 1 full year of Hospice experience.
• First-timers Accepted: No, must have recent specialty experience.

Schedule

• Contract Length: 13 weeks
• Schedule of Assignment Hours: Monday to Friday, 8 AM to 5 PM
• Weekend Requirement: Varies by location and profession.

Certifications Required

• CPR
• BLS

On-Call Requirement

• Varies by location and profession.

Travel Requirements

• Must be willing to travel up to 75 miles from the home clinic.
• Mileage reimbursed at the IRS standard rate.

Local Candidates

• Local candidates are accepted at the same rate.

Licensing

• Pending License Accepted: No, must have license in hand.

Additional Information

• Dress Code: Scrubs
• Charting System: Proficiency in HomeCare HomeBase (HCHB) required within the last 6 months.
• Productivity Expectations: 30 points per week, with each type of visit and activity holding a different point value.

• Background checks
• Health documents (e.g., TB test, vaccinations)
• Various facility-specific documents and agreements
