Required Credentials

1. BLS (Basic Life Support)

10. UHS Ohio Fingerprints

    • Background checks required based on residency duration in Ohio.

11. UHS Education Verification

    • Highest level of education verified; attestations not allowed.

12. UHS Employment Verification

    • Verification of employment within the last 5 years required.

13. UHS Competency Exam

    • Must score at least 80%, completed within 1 year prior to start.

14. Physical/Health Statement

    • Pre-employment exam statement required, completed within one year of initial start date.

15. Influenza Vaccine

    • Proof of vaccination required during flu season.

16. 2-Step PPD or IGRA (Quantiferon/Tspot)

    • Negative reads required within the last 6 months.

17. UHS Criminal Background Check Results

    • Must be completed within 6 months prior to start.

18. UHS Drug Screen

    • Must be completed within 60 days of initial start date.

19. UHS Module Completion Certificate

    • Safety and Compliance Training modules must be completed prior to clearance.
