Responsible for the management and operation of the Department of Pharmacy in accordance with the hospital's stated philosophies, goals, and objectives to ensure the consistent provision of high quality patient care.

Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration.
Unit specific to the contractor’s scope of practice within one year prior to start date; renewed annually
Copies of exams needed, within year of start with passing score of 80% or higher.
HIPAA, Patient Bill of Rights, Safety Orientation, Blood borne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; renewed annually.
Proof of 2 immunizations, positive titer or Declination accepted.
Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870+. Must be completed within 1 year prior to start. Updated annually.
Proof of 3 immunizations, positive titer or Declination accepted.
Proof of vaccine required from October - March. Only Medical/Religious exemptions will be accepted.
Proof of negative Blood test or Skin test within 30 days and the other within 1 year.
Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
