Required Certifications

• BLS (Basic Life Support): Required for onboarding.

License and Health Requirements

• License/Certificate Verification: Primary source verification of professional license and/or certification required within 30 days of start date.
• Fit Test: N95 fit test required within 365 days prior to start date and updated annually.
• MGB Medication Exam: 80% passing score required within 365 days.
• COVID-19 Vaccine: Completed primary series required; no exemptions considered.
• MMR (Measles, Mumps, Rubella): Documentation of vaccinations or proof of immunity required.
• Influenza Vaccine: Required during flu season; no exemptions considered.
• TB Test: Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.
• Health Statement: Signed by MD/DO/NP/PA within 365 days of start date.
• Hepatitis B: Strongly preferred documentation of vaccination or proof of immunity.
• Tdap: Vaccine within 10 years preferred.

• MGB Criminal Background Check: Required for onboarding.
• MGB Massachusetts CORI: Required for onboarding.
• MGB SSN Trace: Required for onboarding.
• MGB National Sex Offender Check: Required for onboarding.
• MGB Exclusion Checks: Required for onboarding.

• MGB Resume: Required for submission, including past 7 years of work experience.
• MGB References: Minimum of 2 recent references required, including at least 1 supervisor.
