Schedule

• Tuesday to Friday: 8:00 AM - 4:30 PM
• Every Other Weekend: 7:00 AM - 3:30 PM (Saturday and Sunday)

Job Description

• OB/GYN experience preferred.

Required Credentials and Health Requirements

1. BLS Certification

   • Required for onboarding.

2. License/Certificate Verification

   • Primary source verification of professional license and/or certification required within 30 days of start date. MA Conditional Licenses not accepted.

3. Fit Test

   • N95 fit test required within 365 days prior to start date and updated annually. No onsite fit testing available.

4. COVID-19 Vaccine

   • Completed primary vaccination series required. No exemptions or waivers considered.

5. MMR Vaccination

   • Documentation of two measles, two mumps, and one rubella vaccine required, or proof of immunity via blood test.

6. Hepatitis B Vaccination

   • Strongly preferred: record of 3 Hep B vaccines or proof of immunity.

7. Influenza Vaccine

   • Required during flu season (September 15 - April 30). No exemptions or waivers considered.

8. TB Test

   • Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.

9. Health Statement

   • Physician statement signed by MD/DO/NP/PA required within 365 days of start date.

10. Tdap Vaccination

    • Preferred: vaccine within 10 years and updated upon expiration.

11. Criminal Background Check

    • MGB Criminal Background Check required.

12. Massachusetts CORI

    • Background check through the Massachusetts CORI system required.

13. Resume

    • Must include past 7 years of work experience and education.

14. References

    • Two recent references required, with at least one from a supervisor or above.

15. Skills Checklist

    • Required for submission.

16. Confirmation of Assignment/POI Form

    • Required for all initial assignments and extensions.

17. Education Verification

    • Verification of highest level of education achieved required.

18. SSN Trace

    • Must verify addresses/aliases for the past 7 years.

19. National Sex Offender Check

    • Required within 30 days of start and updated annually.

20. Exclusion Checks

    • Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists required.

21. TB Symptom Survey

    • Required if positive PPD.

22. Submission Checklist

    • Required for submission.
