Job Description

The Registered Nurse (RN) in Home Health will coordinate and direct the provision of home nursing care according to physician's orders, agency policies, and procedures. Responsibilities include:

Application of the nursing process in patient care.
Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Conducting home visits and attending periodic office meetings.

Patient Care

Average Patients per Day: 5-8
Types of Cases: Start of Care, Wound Care, Follow-up Visits, etc.
Productivity Expectations: 28-30 points per week, with each visit and activity assigned a different point value.

Travel Requirements

Must have reliable transportation and a current driver's license.
Willingness to travel up to 75 miles from the home clinic.
Mileage reimbursed at the IRS standard rate.

Qualifications

Experience Required: Minimum of 1 full year of Adult Home Health or Hospice experience.
Certifications Required: CPR, BLS.
On-call Requirements: Varies by location and profession; to be discussed during the interview.
First-timers Accepted: No, must have recent specialty experience.

Schedule

Typical Hours: Monday to Friday, 8 AM - 5 PM.
Weekend Requirements: Varies by location; may require weekend rotation every 4 weeks or less.
On-call: Yes, every 4 weeks or less.

Additional Information

Dress Code: Scrubs.
Charting System: Proficiency in HomeCare HomeBase (HCHB) is required; must have used it within the last 6 months.
Local Candidates: Accepted at the same rate.

Background checks.
Health documents (e.g., physical exam, TB test).
