Employment Details

• Employment Type: LTA (Long-Term Assignment)
• Contract Length: 13 weeks
• Call Requirement: 2 weekends and 2 weekdays per month (Saturday 0700 to Monday morning at 0700)

Required Credentials

1. BLS (Basic Life Support)

   • Required for clinical positions.
   • Certification from the American Heart Association only.

2. ACLS (Advanced Cardiovascular Life Support)

   • Certification from the American Heart Association only.

3. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are not accepted.

4. MMR (Measles, Mumps, Rubella)

   • Positive titers only from within the last 15 years. Specific booster requirements apply.

5. Fit Test

   • Completed within 1 year of start, updated annually.

6. Hepatitis B

   • Proof of vaccination series or signed declination required.

7. Varicella (Chickenpox)

   • Positive titers only from within the last 15 years. Specific booster requirements apply.

8. Tdap (Tetanus, Diphtheria, Pertussis)

   • Proof of vaccination within the past 10 years.

9. 2-Step PPD or IGRA (Quantiferon/Tspot)

   • Negative reads required within the last 6 months.

10. Physical/Health Statement

    • Completed within one year of initial start date, stating the traveler is able to work without restrictions.

11. Influenza Vaccine

    • Proof of vaccination from October to March or approved exemptions required.

12. Driver's License/State ID

    • Non-expired photo ID required.

Professional Requirements

• UHS I-9 Attestation

  • Completed I-9 or attestation that completed I-9 is on file.

• UHS Education Verification

  • Highest level of education verified; attestations not allowed.

• UHS Employment Verification

  • Verification of employment within the last 5 years required.

• UHS Criminal Background Check Results

  • Ran within 6 months of start, including SSN Trace and County searches.

• UHS OIG Search

  • To be completed within 6 months prior to start.

• UHS SAM/EPLS/GSA/OFAC

  • Supporting documentation that the individual was screened and cleared.

• UHS Drug Screen

  • Must be completed within 60 days of initial start date.

• UHS Module Completion Certificate

  • Safety and Compliance Training modules must be completed prior to clearance.
