Required Credentials and Qualifications

Licenses and Certifications

• License/Certificate Verification:

  • Primary Source Verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are NOT accepted.
• ARRT Certification:

  • Required for the position.

Health and Safety Requirements

• COVID-19 Vaccine:

  • Proof of vaccine series or completion of client medical or religious exemption form preferred.
• MMR:

  • Positive MMR titers ONLY from within the last 15 years. Specific booster requirements apply.
• Fit Test:

  • Completed within 1 year of start, updated annually.
• Hepatitis B:

  • Proof of full series or at least two vaccines from the series, or signed declination.
• PPD/TB Testing:

  • 2-Step TB test with negative reads within the last 6 months. If positive, a clear chest x-ray and current TB questionnaire are required.
• Varicella:

  • Positive Varicella titers ONLY from within the last 15 years. Specific booster requirements apply.
• Tdap:

  • Proof of vaccination within the past 10 years; declinations not accepted.
• Influenza Vaccine:

  • Proof of vaccination from October to March or approved exemptions required.

Background Checks

• UHS Criminal Background Check Results:

  • Must be completed within 6 months prior to start.
• UHS OIG Search:

  • To be completed within 6 months prior to start, updated annually.
• UHS SAM/EPLS/GSA/OFAC:

  • Supporting documentation required, to be completed within 6 months prior to start.

Additional Requirements

• Education Verification:

  • Highest level of education verified; attestations not allowed.
• Employment Verification:

  • Must provide verification of employment within the last 5 years.
• Physical/Health Statement:

  • Completed within one year of initial start date, stating the traveler is able to work without restrictions.
• UHS Skills Checklist:

  • Clinical skills checklist required to be completed within 1 year prior to start.
• UHS Competency Exam:

  • Must score at least 80%, to be completed within 1 year prior to start.
• UHS Core Competencies:

  • 80% passing score required, completed within 1 year prior to start.

  • Must include education and work history, along with 2 professional references required within the last 2 years.
• UHS Submission Packet:

  • Job application to be completed by the candidate.
• UHS Badge Photo:

  • Shoulders up, plain background, headshot required.
