Must have page 27 signed form on file in Trinity VMS (Trinity Code of Conduct Form)
Must complete standard I9
Confidentiality of Patient and Proprietary Information and Information System Security Agreement required
Trinity Integrity and Audit Services Form completion
Trinity Specialty Specific Competency Testing within a year
2 Step PPD or IGRA (Quantiferon/Tspot) required
Trinity Specific Covid Questionnaire must be completed
Skills Checklist within a year, 2 Professional References (Trinity Profile) for submission
Trinity Conflict Of Interest Disclosure Form must be signed
Trinity Criminal Background Check within 30 days
Education Verification (degree or GED) by Trinity at onboarding
EPLS verification within past 30 days
GSA/OIG verification within past 30 days
NSO verification within past 30 days
Trinity System Access Form completion one week before start
