Certification details
- Candidates with an AZ home address on their drivers license will not be considered - please include a copy in submission. Start Date should be on or very quickly after the Order Start Date listed. All RTO should be listed on the cover page, must be less than 7 days.
Job details
- 24 months minimum of current PCU experience - REQUIRED.
- Travel experience - Preferred.
- Banner Cover Page completely filled out.
- Resume with all facilities, facility size, beds, trauma level, and units listed.
- Skills Checklist specific to the discipline (must be less than a year old).
- Active License (verification must be less than 30 days old).
- Learning modules must be completed prior to start of assignment.
Job requirements
- RTO will not be approved in the first week of assignment or holidays.
- All shifts the traveler is open to working on should be listed on the submission.
Schedule information
- Department: PCU
- Experience in departments that candidates are willing to ACCEPT/work in should be noted.
Unit specific information
- Flu vaccination - Banner declinations can be applied for (reach out for forms).
- Please add vaccination status in the submission notes.
- Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
- Banner has very specific background requirements that must be adhered and attested to.
