Certifications Required

• ACLS - Advanced Cardiac Life Support (AHA ONLY)
• BLS - Basic Life Support (AHA ONLY)
• PALS - Pediatric Advanced Life Support (AHA ONLY)
• NRP - Neonatal Resuscitation Procedure (as applicable)
• CHEMO - Chemotherapy (as applicable)
• TNCC - Trauma Nursing Core Course (as applicable)
• EFM - Electronic Fetal Monitoring (as applicable)
• HIPAA Safety Manual Acknowledgement (required annually)
• NIH Stroke Certification (as applicable)
• Medication Test/Pharmacology Competency Exam (as applicable)
• Competency Exam (as applicable)

Health Documents Required

• Physical Exam - required within 1 year of start
• TDAP Vaccine - required within 10 years of start
• Negative TB or Chest X-Ray - required
• TB Questionnaire - required for all candidates
• Rubeola (Measles) - 2 vaccines or positive titer required
• Mumps - 2 vaccines or positive titer required
• Rubella - 2 vaccines or positive titer required
• Hepatitis B - positive titer, series (3), or declination required
• Varicella - 2 vaccines, positive titer, or history of chickenpox required
• Drug Testing - negative 10 panel drug screen within 1 year of start
• Flu Vaccine - or declination required during flu season
• COVID-19 Vaccine - proof of vaccination or declination required

Background and History Requirements

• Background Check - required and must be renewed annually
• EPLS/GSA/SAM Check - required within 30 days of start
• HHS/OIG Check - required within 30 days of start
• References - 2 required within 3 years of start
• Employment Application - copy required
• Skills Checklist - required and updated annually
• State Specific Search - TX DADS Check within 30 days of start
