Required Credentials and Certifications

1. BLS (Basic Life Support)

   • Must be AHA certified.
   • Required for onboarding.

2. Nursys Verification

   • Required for submission within 30 days prior to start.
   • Must complete FACIS Questionnaire for any disciplinary actions.

3. License/Certificate Verification

   • Required for submission within 30 days prior to start.

4. Ascension Medical Surgical Exam

   • Must be completed prior to start and updated every 3 years.
   • Minimum 80% score required; only 2 attempts allowed.

5. Ascension Color Vision

   • Must be completed prior to start and updated every 3 years.

6. Ascension Core Competencies

   • Must be completed within a year of start and updated yearly.
   • Minimum 80% score required; only 2 attempts allowed.

7. NIH Certification

   • Group A updated yearly; Group B-F updated every 2 years.

8. Ascension EKG Exam

   • Must be completed prior to start with a passing score of at least 80%.

9. Ascension Onboarding Requirements Education Attestation

   • Must be uploaded by the agency after completion of pre-hire modules.

10. MMR (Measles, Mumps, Rubella)

    • Documented proof of two vaccines or positive IGG titer required.

11. Fit Test

    • Required for all travelers; preferred to be completed prior to start.

12. Hepatitis B

    • Documentation of vaccination series completion or signed declination required.

13. Influenza Vaccine

    • Required for flu season (October 1, 2024 - March 31, 2025); exemptions accepted with mask requirement.

14. Health Statement

    • Statement of good health or completed physical exam required prior to first assignment.

15. Driver's License/State ID

    • Must match address on file and be valid.

16. 2 Step PPD OR IGRA (Quantiferon/Tspot)

    • Required prior to start; updated annually.

17. Varicella

    • Documented proof of vaccination or history required.

18. Tdap

    • Must be valid for 10 years from administered date.

19. Ascension Profile

    • Resume or profile required for submission.

20. Ascension 2 Professional References

    • At least two professional references required.

21. Ascension Criminal Background Check Results

    • Must be completed within 30 days of first assignment.

22. Ascension EPLS

    • Must be run within 90 days of start.

23. Ascension Skills Checklist

    • Clinical skills checklist to be completed within one year of start.

24. Ascension 12 Panel Drug Screen

    • Must be completed within 30 days prior to start.

25. Ascension Badge Photo

    • Forward-facing headshot required.

26. Ascension Education Verification

    • Verification of education relevant to clinical work required within 90 days of start.

27. Ascension Confirmation of Assignment

    • Must be provided upon initial assignment and updated upon each extension.

28. Ascension Employment Verification

    • Third-party employment verification for the past seven years required.
