Trinity Code of Conduct Form, Confidentiality agreement, and Integrity and Audit Services Form needed
Trinity Specialty Specific Competency Testing and Color Vision required
COVID-19 Vaccine proof, declination or exemption

Weekend Package (Friday 7pm-7am)

American Heart Association certification required for direct patient care personnel
