Health Document Requirements

Tetanus/Diphtheria/Tdap: Vaccine within 10 years
Negative TB or Chest X-Ray: Required within 3 months of start
TB Questionnaire: Required annually
MMR Vaccines: 2 vaccines or positive titer required
Hepatitis B: Series, titer, or declination
Varicella: 2 vaccines or positive titer required
Drug Testing: Required within 30 days of start
Flu Seasonal Vaccination: Required from Nov 1 – March 31

Background Check

Required within 6 months of start covering all locations lived/worked/in school within the past 7 years.

Additional Information

First-time travelers are not accepted.
Scrubs: Navy blue or white.
