Required Credentials and Health History

1. COVID-19 Vaccine

   • Proof of vaccine and 1 booster or exemptions must be submitted for approval.

2. MMR (Measles, Mumps, Rubella)

   • Proof of 2 immunizations, positive IGG titer, or declination accepted.

3. Hepatitis B

   • Proof of 3 immunizations and positive antibody titer, or declination accepted.

4. TB Test

   • Must be within 30 days of start. Negative results required. Updated annually.

5. Health Statement

   • Statement signed and dated within 1 year of start stating traveler is able to work without restrictions. Updated annually.

6. Driver's License/State ID

   • Required for onboarding.

7. Varicella (Chickenpox)

   • Proof of 2 immunizations, positive titer, or declination accepted.

8. Tdap (Tetanus, Diphtheria, Pertussis)

   • Proof of vaccination within the past 10 years or declination accepted.

9. Drug Screen

   • Must be within 30 days of start, 10 panel standard test. Updated annually.

Professional Requirements

1. CHS Profile

   • Resume or profile required for submission.

2. Professional References

   • 2 professional references required, with at least 1 supervisory reference.

3. Criminal Background Check

   • Must be completed within 30 days of start and updated every 3 years.

4. OIG, EPLS, OFAC, and National Sex Offender Searches

   • Must be completed within 30 days of start and updated annually.

5. Medi-Cal Suspended and Ineligible Provider List

   • Must be checked within 30 days prior to the first assignment and updated with each extension.

6. Education Verification

   • A diploma, degree, or transcripts required, along with primary source verification of the highest level of education.

7. License/Certificate Verification

   • Primary source verification of professional license and/or certification required.

Certifications

1. BLS (Basic Life Support)

   • AHA or ARC certification required.

2. ACLS (Advanced Cardiovascular Life Support)

   • AHA or ARC certification required.

3. PALS (Pediatric Advanced Life Support)

   • Required.

4. NRP (Neonatal Resuscitation Program)

   • Must be completed through AAP NRP learning platform.

5. TNCC (Trauma Nursing Core Course)

   • Optional certification.

6. ENPC (Emergency Nursing Pediatric Course)

   • Optional certification.

7. CHS Skills Checklist

   • Skill-specific checklist required to be completed within 1 year of start.
