Competencies Required

• Medication Test/Pharmacology Competency Exam
• Competency Exam

Health Documents Required

• Physical Exam - within 1 year of start
• TDAP - required within 10 years of start
• Negative TB or Chest X-Ray - within 1 year of start
• Rubeola (Measles) - proof of Titer or 2 doses of MMR for candidates born in 1957 or later
• Mumps - proof of Titer or 2 doses of MMR for candidates born in 1957 or later
• Rubella - proof of Titer or 1 Vaccine for candidates born in 1957 or later
• Hepatitis B - Titer, Declination, or Vaccine Series
• Varicella - Titer, 2 vaccines, or History of having Chicken Pox
• Drug Testing - 9 Panel Drug Screen within a year of start
• Flu Seasonal Vaccination/Documentation - required during Flu season (10/1 - 3/31)

Background and History Checks

• Background Check - standard, updated annually
• EPLS/GSA/SAM - required annually
• HHS/OIG - required annually
• References - 2 Professional References required (within 3 years of start)
• Skills Checklist - required annually

• Government Issued Photo ID required.
• Confidentiality of Information Policy Acknowledgement - must be reviewed/signed prior to start.
