Position Summary: The Physical Therapist is responsible for evaluating, planning, directing, and administering of Physical Therapy services as prescribed by a licensed Provider.

Treatments provided are to restore function, improve mobility, relieve pain, prevent/limit physical disabilities and/or increase independence to improve patients' overall functioning within their environment.

Skilled therapy techniques

Collaborates with Rehab team

Supervises and directs Physical Therapy Assistants (PTA); maintains open communication with members of the Rehab team.

Performs appropriate assessment on all patients as related to physician order; designs treatment programs.

Formulates realistic and appropriate short and long term treatment goals; includes patient/family in development of plan of care.

Observes and evaluates treatment effect; recommends changes to physician as necessary.

Ensures accurate patient charges and documentation; complies with regulatory standards.

Provides information related to departmental budget; ensures optimization of patient care and service.

Demonstrates thorough knowledge of computerized documentation; complies with all established safety procedures.

Participates in performance improvement activities.
