    Job requirements
  • Education required: Bachelor’s degree in Physical Therapy from an American Physical Therapy Association and an American Medical Association accredited program required.
  • Work Experience: Minimum one (1) year experience in an acute care hospital setting. Minimum one (1) year experience in a rehab facility or equivalent serving the adult population. Six (6) months home care experience preferred. Computer skills preferred.
    Schedule information
  • Phelps can call off 1 shift per 30 days without penalty
  • GSA hours are not guaranteed the week of Orientation. Traveler is guaranteed set hours per week, except during holidays where unit is closed*
    Additional Details
  • The Homecare PT develops a plan of care for each patient assigned after evaluation/assessment to meet the patients and physicians goals of care, specific to the patient. PT may be assigned home health or hospice patients. PT reports to the Hospice Interdisciplinary Team regarding care and treatment of other team members as assigned to develop an interdisciplinary plan of care for each patient. Supervises PTA as assigned.
