  • Responsible for daily operations of the case management department.
  • Supervision of the delivery of initial psychosocial assessments and follow-up assessments for discharge planning.
  • Maintains current knowledge of resources available within the community.
  • Ensures the accurate and timely implementation and evaluation of new and existing programs.
  • Engages and evaluates competency-based performance of staff members.
  • Partners with key stakeholders to provide daily review for complex or catastrophic cases and treatment plans.
  • Provides education to the case management staff on performance improvement, plans and organizes orientation, and in-service training for staff members.
  • Collaborates with the Manager to build, monitor and evaluate collaborative work processes among case managers.
  • Monitors timely accurate processing by the case managers to post levels of care such as infusion services, homecare agencies.
  • Implements and evaluates effectiveness of departmental performance.
  • Participates in unit rounding twice weekly to assist with complex social issues and develop appropriate discharge options.
    Job requirements
  • Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the HFHS’ Customer Service Policy.
  • Must practice customer skills as provided through on-going training and in-services.
  • Must possess personal qualities including being self-directed, flexible, committed to the team concept, demonstrate teamwork, initiative, willingness to learn, open to new learning experiences, accepts and respects diversity without judgment, and demonstrates customer service values.
