    Additional Details
  • Government Issued Photo ID required.
  • Driver’s license required to be provided within 48 hours of signing.
  • If traveler does not provide within 48 hours or provides a driver’s license that does not match their RN license, please start an email chain with appropriate personnel to get proof of residency on file asap.
  • Facility does not care about a space in the name on the driver’s license.
