Required Certifications and Credentials

1. Basic Life Support (BLS)

4. Fit Test

   • Required for onboarding.

5. Trinity Code of Conduct Form

   • Standard I9 completed by the agency.

7. Trinity Confidentiality Agreement

   • Must have page 5 signed and on file.

8. Trinity Integrity and Audit Services Form

   • Required for onboarding.

9. Trinity Specialty Specific Competency Testing

   • Attach copy of specialty exam completed within a year.

10. Trinity Child and Adult Dependent Abuse Training

    • Required for Iowa.

11. Trinity IA Sing Report

    • Criminal history check required within 30 days prior to start.

12. Trinity OFAC

    • Must be completed within 30 days of start.

13. Trinity Skills Checklist

    • Required for clinical contractors.

14. Driver's License/State ID

    • Must be dated within 30 days of start.

    • Attach completed report of highest level of academic education.

    • Attach scanned report dated within the past 30 days.
