Qualifications

• Experience: Must have Oncology experience
• Certifications Required:

  • BLS (Basic Life Support)
  • RPh (Registered Pharmacist)
  • State License
  • Chemo Certification

  • Acknowledgement of Pass/Fail Testing
  • State License Verification
  • NURSYS Verification (RN, LPN/LVN, AP)
  • Management of Assaultive Behavior (MAB)
  • NIH Stroke Certification
  • Pass/Fail Testing (EKG/Medication)
  • CPI (Crisis Prevention Institute) Certification
  • Medication Test/Pharmacology Competency Exam
  • Color Blind Testing (for OR and Cath Lab positions)

Health Documents

• Physical Exam: Must be within 1 year of start
• Fit Testing: N95 only, within 1 year of start
• Vaccinations:

  • Tetanus/Diphtheria/Tdap
  • Negative TB or Chest X-Ray
  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug Testing: Must be completed within 30 days of start
• Flu Vaccination: Required during flu season (Oct 1 – Apr 30)

Background Check

• Must be started within 30 days of the assignment start date
• Includes searches for Pima County, AZ, and other relevant checks

References

• 2 professional references from employment within the last 3 years required, with 1 from a supervisory role.
