Job Description

• Role: Therapy PT Home Health
• Responsibilities:

  • Conduct home visits for patients.
  • Participate in periodic office meetings.
  • Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Average Patients per Day: 5-6
• Points System:

  • 30 points per week, with each type of visit and activity (documentation, travel, etc.) holding a different point value.
  • Contractors will be assigned between 5-10 patients per day, equating to roughly 6-8 points.
• Types of Cases: Start of Care and Follow Ups.

Requirements

• Experience: Minimum of 1 full year of Adult Home Health Experience.
• First-timers Accepted: No, must have recent Home Health experience.
• Certifications Required: CPR, BLS.
• On-call Requirement: Varies per job/location; to be discussed during the interview.
• Weekend Requirement: Varies per location and profession; to be discussed during the interview.
• Territory: Must be willing to travel up to 75 miles from the home clinic.
• Mileage Reimbursement: IRS Standard rate; travel is compensated from patient home to patient home.

Additional Information

• Local Candidates Accepted: Yes, at the same rate.
• Pending License Accepted: No, must have license in hand unless it takes a few days to obtain.
• EMR System Used: HCHB.
• Weekly Units of Productivity Expectations: 28-30.

Compliance and Documentation

• Candidates must complete various compliance and training documents prior to starting, including:

  • Acknowledgment of policies and procedures.
  • Health documents (e.g., physical exam, TB test, vaccinations).
  • Background checks and reference checks.

Health Requirements

• Physical Exam: Must be completed within 12 months of start.
• TB Testing: Negative PPD, IGRA, or Chest X-Ray required within 12 months of start.
• Vaccinations: Proof of MMR, Hepatitis B, Varicella, and COVID-19 vaccination required.
