As a Physical Therapist in a Skilled Nursing Facility, you will be essential in providing high-quality patient care and rehabilitation services. Reporting to the Rehabilitation Director, you will utilize your core skills in anatomy, physiology, and medical terminology to develop effective care plans. Duties • Conduct thorough evaluations of patients to assess their physical condition and functional abilities. • Develop individualized treatment plans based on patient assessments, goals, and needs. • Implement therapeutic interventions including manual therapy techniques, exercise programs, and modalities to promote healing and recovery. • Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes. • Educate patients and their families on rehabilitation processes, home exercise programs, and lifestyle modifications to support recovery. • Collaborate with healthcare professionals to coordinate comprehensive patient care, including discharge planning and follow-up services. • Maintain accurate documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with regulatory standards. • Stay current with advancements in physical therapy practices and participate in ongoing professional development. Qualifications • Valid Physical Therapist license • Strong patient care and assessment skills • In-depth knowledge of anatomy and physiology • Proficiency in medical terminology and physical therapy techniques • Experience with discharge planning and care plans • Familiarity with acute care settings and geriatrics preferred
