Certs: BLS License: RN License Experience: 2 years of experience MUST have Home Health experience MUST have HCHB experience Must have OASIS experience Oasis E - after Jan 2023 experience is required No more than 1 year gap All documentation due same day Homecare Homebase (Productivity points, minimum average 26-28) That could mean 3 people or 7 people About 20-25 cases per week. 5-7points per day Absolute must - true home health experience, Oasis documentation Overview: Facility Name: AccentCare Golden Valley/Brooklyn Park Location: Varies Minneapolis/St Paul Facility size: Varies - home-based care Scrub Color Requirement: any color PPE: TBD Parking: TBD Pay Cycle: Monday-Sunday EMR: Must have OASIS experience Case types: Homecare Home Health RN Orientation: About 2-3 days of orientation. Modules, get an intro to the team Other Details: Once a week they have to go into the office (Usually Tue, Wed and Thur). Mandatory in person Hours: Branches are open 8am - 5pm, they may have patients who want to be seen before or after. Homecare Homebase MUST be completed at the patients home. Absolute must - true home health experience, Oasis documentation
