Job ID #16237752 BCV 1. Driving radius for Home Health position? 40 mile radius 2. Cases per day? up to 5 in single family homes, apartments or assisted living facilities about 30 to 40 mi radius. A majority of the patients will be right in the Fargo Moorhead area though. 3. Will Therapist need to cover SNFs? If so, which locations? no 4. Will Therapist need to supervise PTAs? If so, how many? yes, one 5. Software used: Remote Client/Epic 6. Will train New Graduate? Yes 7. Productivity expectations? 85-90% once trained 8. Guarantee: 40 hours per week 9. Targeted Start Date: as soon as possible 10. Mileage and drive time billable? reimbursed- yes Essential Duties & Responsibilities Assesses clients’ needs and develops plans for therapy that meets identified needs. Administers and performs therapeutic interventions with clients. Maintains clients’ self-esteem, privacy and confidentiality of personal information. Provides clients with opportunities for independence and choice consistent with their care plans. Notifies physician of changes; processes orders. Documents observations and services provided. Provides direct consultation with physician and/or interdisciplinary team members; receives and shares information. Provides training on programs, adaptive equipment and other interventions. Utilize and supervise therapy assistants as outlined by Federal and State regulations and documents appropriately in the clinical record. Utilizes appropriate bag techniques and standard precautions. Follows infection control and safety control standards when using equipment. Basic Responsibilities: This position will be held accountable for complying with all related laws, regulations, company policies and procedures pertaining to his or her position and for fulfilling his or her obligations under the organization's corporate compliance program. Certification: Completion of an accredited physical therapy program required. Prior work experience as a therapist in a clinical setting helpful. National registration and licensure for the state of practice, if required.
