Job Description: • Title: Home Health LPN/LVN • Start Date: 04/21/2025 (Must be within 4 weeks) • Shifts: Day 5x8-Hour (08:00 - 16:30) • Duration (weeks): 13 weeks • Guaranteed Hours: 40 • RTOS: No more than 5 days of RTO accepted. Requirements: • 2 YR of Skilled Home Health Experience required. • Previous travel experience required. • BLS/ BCLS required. Special Requirements: • Traveler is expected to have their own transportation. • System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles. • Facility requires primary series COVID vaccination in addition to one booster. • Oasis is required. Please do not apply if don’t have Oasis Experience.
