Key Responsibilities: - Assess & Treat: Conduct comprehensive evaluations and create individualized treatment plans. - Therapeutic Interventions: Provide physical therapy treatments, exercises, and modalities to enhance mobility and function. - Educate & Support: Teach participants and families about treatment plans and home exercises. - Document Progress: Maintain accurate records using EMRs like Care Compass. - Collaborate: Work closely with a multidisciplinary healthcare team to provide holistic patient care. - PACE Compliance: Ensure all care meets Program of All-Inclusive Care for the Elderly (PACE) regulations. - Home Visits: Travel as needed to participants' homes, nursing facilities, and other care sites. Qualifications: ✔ New York State licensure as a Registered Physical Therapist. ✔ 1+ years of experience (geriatrics preferred). ✔ Bi-lingual preferred (based on participant demographics). ✔ Physical ability to lift up to 50 lbs, transfer participants, bend, kneel, reach, and stand for extended periods. ✔ Strong interpersonal skills to build rapport with participants and staff. ✔ Knowledge of EMRs (Care Compass preferred).
