Job Title: Social Worker - LCSW (Licensed Clinical Social Worker) Location: Providence Swedish Medical Center - Edmonds Campus, 21601 76th Ave W, Edmonds, WA 98026-7506 Unit: Case Management Shift: Day 5x8-Hour (08:00 - 16:30) Contract Duration: 13 Weeks Start Date: April 21, 2025 Compensation: • Weekly Gross: $2,403.00 Per Week • On W2(Taxable): $800.00 Per Week ($20/hour) • Stipend (Non-Taxable): $1,603.00 Per Week Job Description: We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join the Case Management team at Providence Swedish Medical Center - Edmonds Campus. This role will focus on providing case management services, discharge planning, and utilizing best practices for patient care coordination. The ideal candidate will have experience in acute care hospitals, case management, and discharge planning, as well as familiarity with tools like Epic and Cerner for documentation. Key Responsibilities: • Perform comprehensive case management and assessments for patients, ensuring smooth coordination of care. • Develop and implement individualized care plans and assist in discharge planning. • Conduct biopsychosocial assessments, utilizing various criteria such as InterQual and Milliman guidelines. • Ensure all documentation is completed according to the plan of care and reflects coverage criteria. • Facilitate patient transitions to post-acute care settings, including rehabilitation and long-term care facilities. • Float to other units as needed within the scope of practice. • Participate in care conferences and collaborate with interdisciplinary teams. • Provide crisis intervention and address high-risk patient safety concerns as needed. • Ensure compliance with regulatory requirements, including CMS, HIPAA, ICD-10 coding, and The Joint Commission standards. Requirements: • Licensure: Washington-issued post-MSW license (such as LCSW or LCISW) required. • Experience: Minimum 2 years of case management and discharge planning experience in an acute care setting. Experience in Utilization Management is a plus. • Certifications: BCLS (Basic Life Support) certification required. Skills: • Strong knowledge of case management, care coordination, and discharge planning. • Familiarity with CMS, HIPAA, ICD-10, CPT coding, and regulatory guidelines. • Experience with major disease processes such as dementia, stroke, cancer, CHF, COPD, diabetes, HIV/AIDS, etc. • Experience using Electronic Health Records (EHR) systems like Epic or Cerner. • Familiarity with Advance Directives, cultural issues, and high-risk patient safety. Other Requirements: • Floating: All travelers are expected to float to other units within their scope of practice if needed. Floating to other facilities within a 40-mile radius may occur. Travelers are responsible for transportation when they float to other campuses. • The candidate must reside at least 100 miles away from the facility.
