Speech Language Pathologist – Rehabilitation Services (Mixed Settings: Outpatient, Inpatient, and Home Health/Hospice) Position Summary: As a Speech Language Pathologist you will have a unique opportunity to work in three disciplines: outpatient, inpatient, and home health/hospice and help grow these programs! You will join rehabilitation and home health departments and work with SLP, occupational and physical therapy services, nursing, and home health aides. In all settings you will work 1 on 1 with your patients and will have time built into your schedule for documentation. In home health/hospice, we own a fleet of vehicles that are available to the part-time and full-time staff. The facility values a healthy work/life balance for our employees and high-quality care for our patients. We encourage continuing education with a stipend each year and trainings built into our therapist’s schedules. Our home health/hospice and rehabilitation departments have a close relationship and strongly believe in an Interdisciplinary Team approach to our patient care. We are looking for an SLP who is passionate about their profession and growing access to SLP services in Grant County. Our ideal candidate will have experience in each of these settings between their work and/or clinical education. Speech Language Pathologist will perform duties as assigned for improving a patient’s function in speech/language, oral/fascial musculature and structure, cognitive skills, voice, fluency, breathing and/or swallowing abilities to maximum potential. The SLP will provide this treatment per physician’s order and coordinate therapeutic activities with the patient, caregiver, and other health care disciplines. SLP will be a part of the interdisciplinary team within Blue Mountain Hospital District’s inpatient, outpatient, home health, hopsice, and transitional care programs. S/he will follow BMHD and the department guidelines. Required: • Bachelor’s Degree & Master’s Degree • Graduate of an accredited school of speech language pathology • Basic Life Support (BLS) certified by American Heart Association (AHA). • SLP license, in good standing with State of Oregon Preferred: • Working experience in home health. hospital inpatient and outpatient settings #HC8 43
