Job ID:31033316 Position Overview: The Licensed Practical Nurse (LPN) at Troy Hills Center provides high-quality care to our residents, focusing on their comfort, safety, and well-being. Working alongside a team of RNs and healthcare professionals, you’ll care for patients in need of skilled nursing, rehabilitation, and long-term care. You’ll help ensure that every resident receives the attention and treatment they deserve. Key Responsibilities: Resident Care: Administer Medications: Give prescribed medications to residents, ensuring the right dose and timing. Monitor Vital Signs: Regularly check residents' vital signs, including temperature, blood pressure, and heart rate. Assist with Daily Activities: Help residents with daily tasks such as bathing, dressing, eating, and moving around. Wound Care: Assist with cleaning and dressing wounds, and monitor for signs of infection. Provide Comfort and Emotional Support: Ensure that residents feel cared for and comfortable. Offer support to both residents and their families. Documentation and Reporting: Record Care Information: Document vital signs, medications, and any changes in residents' conditions. Report Changes: Notify the RN or doctor about any significant changes in residents’ health. Team Collaboration: Work with the Healthcare Team: Collaborate with doctors, nurses, and other team members to ensure the best care for residents. Communicate with Families: Keep families updated on their loved ones’ health status and any important changes. Infection Control and Safety: Follow Safety Guidelines: Ensure that residents are safe, minimizing risks like falls and infections. Use Protective Equipment: Follow infection control guidelines and use personal protective equipment (PPE) as required. Qualifications: Education: Must be a graduate of an accredited LPN program and have a valid LPN license in NJ. Experience: Prior experience in long-term care or rehabilitation is a plus. Skills: Strong communication skills. Ability to provide compassionate, patient-centered care. Proficiency in nursing tasks like administering medications and monitoring vitals. Ability to maintain accurate records. Physical Requirements: Must be able to assist residents with movement and physical tasks. Other Requirements: Certifications: CPR/BLS certification (or willing to obtain). Background Check: Must pass a criminal background check and drug screening. Vaccination Compliance: Must meet facility health and COVID-19 vaccination requirements.
