Key responsibilities may include: Patient Evaluation: • Performing thorough initial and ongoing assessments of patients with complex medical needs, including detailed functional evaluations to identify limitations and treatment goals. Treatment Plan Development: • Creating individualized treatment plans based on patient needs, incorporating a variety of therapeutic interventions like manual therapy, therapeutic exercises, and modalities like electrical stimulation. Hands-on Therapy: • Implementing manual therapy techniques to address pain, improve mobility, and restore function in patients with complex conditions. Patient Education: • Providing clear instructions and education to patients and their families regarding exercise routines, self-management strategies, and home care practices to promote optimal recovery. Collaboration with Healthcare Team: • Working closely with other healthcare professionals like physicians, nurses, and case managers to coordinate patient care and ensure seamless transitions. Progress Monitoring and Adjustment: • Regularly evaluating patient progress, making necessary modifications to treatment plans as needed, and documenting treatment outcomes. Discharge Planning: • Participating in discharge planning, including recommending appropriate assistive devices and follow-up care options for patients. Quality Improvement: • Contributing to quality improvement initiatives within the department by identifying areas for improvement and implementing evidence-based practices. Required qualifications: • Doctorate of Physical Therapy (DPT) degree • Current state licensure as a physical therapist • Experience working with complex patient populations, such as post-surgical patients, individuals with neurological conditions, or critically ill patients • Strong clinical reasoning and problem-solving skills • Excellent communication and interpersonal skills to effectively interact with patients, families, and other healthcare providers
