Licensed Social Worker – Home Health for the PACE/LIFE Program - mostly in home visits- some in center. PACE - Program of All-Inclusive Care for the Elderly Life - Life Independence for the Elderly Job Sepcifics: 40 hours per week with hours from 8:00 am to 4:30 pm No on-call duties or weekend work required Work involves both center-based and community-based tasks Coverage area includes Mercer County and North Burlington County Regular visits to participants' homes Responsibilities include conducting assessments for the development of care plans and collaborating with families Requirements: Experience with the senior demographic is mandatory and must be address in the work history 3-4 years of homecare experience preferred Experience in addressing challenging social and psychological needs, particularly in the community setting Background in psychology is a significant advantage Active LSW Certification Flu vaccination is mandatory
