Active MA license required at time of submissions Local candidates not accepted JOB REQUIREMENTS: KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED 1. Master's degree in social work from an accredited school. 2. Minimum 2- years of experience in health care, with special interest and understanding of mental health diagnosis, substance issues, and interpersonal/domestic abuse. 3. LCSW licensure with plan for LICSW. 4. Excellent interpersonal and organizational skills. Strong computer skills. Demonstrated ability to work collaboratively in interdisciplinary teams. 5. Ability to exercise sound clinical judgement in fast-paced and demanding settings. The Clinical Social Worker (CSW) is responsible for assisting patients and their families or significant others with the stress and difficulties related to illness, personal issues, financial difficulties, and other emotional/mental health issues. The social worker’s interventions take into consideration the patient’s strengths, resources, insurance coverage, family system, and motivation for change. The clinical social worker collaborates with nurse care managers and other members of the multidisciplinary team to facilitate safe and effective discharge plans. The social worker supports the mission of the Hospital and participates in the Hospitals’ performance improvement and patient safety initiatives. For Wyman specifically, CSW must provide integrated, interdisciplinary geriatric assessment and care. Must have a passion for working with elders who may be more vulnerable as a consequence of advanced age, or medical comorbidities and issues related to aging. The ability to develop trusting relationship with diverse patients struggling with mental health and/or substance use disorders. The ability to develop trusting relationships with family members is an essential component of the position. Principle duties and responsibilities included but not limited to the following: 1. Perform biopsychosocial evaluations in accordance with departmental policies and standards. 2. Document notes related to ongoing work in a timely and comprehensive manner. 3. Collaborates with MD in planning and participating in family meetings. 4. Provides support and education to patients and families related to illness, hospitalization, and post discharge options and follow up. 5. Assists in facilitating the continuum of care and accessing community services for patients and families as needed. 6. Completes insurance reviews as indicated. 7. Utilizes supervision by bringing appropriate materials for discussion and following up on recommendations. 8. Participates in daily rounds and consults with other clinical staff to facilitate enhanced patient care.
