Job Summary:
COVID-19 vaccination is recommended but not required. Flu vaccination will be required October through April. Flu exemption applications must be received and approved by Phoenix Children’s prior to starting. Anyone not vaccinated for either Flu or COVID-19 will be required to sign Declination & Masking Protocol agreements and must mask during the full respiratory virus season.
Job Summary:
This position provides occupational therapy services to a pediatric population, neonate to adolescent, including evaluation and treatment in independent living, sensory-motor integration, cognitive and psychosocial skill.
Education Requirements:
Graduation from an accredited Occupational Therapy program
Experience Requirements:
1 year Pediatrics work experience (school experience will not be considered)
Ortho, neuro, developmental pediatric experience preferred
Certification/License/Registry Requirements:
Certification with the National Board for Certification in Occupational Therapy (NBCOT) prior to start date
State of Arizona Occupational Therapist license prior to start date
Current AZ Department of Public Safety Fingerprint Clearance Card prior to start date
Current BLS certification from the American Heart Association prior to start date
