Schedule: o M,W,F 36 to 40 hours o 4:30AM-5:00PM or 6:00Am-6:30PM o Subject to change but never will have weekends or overnights Pay Rate: o 1 year experience / No Dialysis Exp (36-38 hour) – 78-79K o 2-5 years’ experience (40-45 hour)- 83K-94K o 5-9 years’ experience (45-50 hour) - 94K-104K o 9+ years’ experience (50-55 hour)- 104K-115K o In Alaska, over time is considered after 8 hours a day so OT is available! Must Haves: - RN License o Alaska License or willing to receive it - Graduate of an accredited school of Nursing - CPR Certification - Minimum of one year of nursing Experience ( ex: Internal Medicine, Med- Surg, or Dyalisis) Plusses: - CNN/CDN Credentials Job Description: Responsible for the delivery of safe, effective nursing care provided for a primarily adult/geriatric end stage renal disease patient population. The RN will utilize nursing processes to assess individual patient needs, determine and implement a plan of care, evaluate and revise plan according to changing patient care needs. Initiates and provides on-going patient and family education and includes patient/family in care planning, and documenting care provided. The RN also coordinates the efforts of members of the interdisciplinary team to ensure continuity of care for each patient. Day to Day: - Responsible for all aspects of direct patient care, including, but not limited to new patient admissions; patient transfers and discharges; nursing assessments (both initial and pre/post each treatment); initiation, monitoring and terminating dialysis. - Rounds on all assigned patients to ensure their needs are met; and to ensure that staff is delivering physician prescriptions to all patients. o Routinely rounds with physicians to ensure coordination of care. o Uses rounding opportunities to mentor dialysis technicians in all aspects of clinical management of the dialysis patient. - Ensures plan of care is developed for all assigned patients to ensure they achieve the desired clinical outcomes. - Reviews the data collected by the dialysis technicians and assesses each assigned patient pre and post dialysis and ensures the patient is provided a treatment appropriate to meet their individualized care plan. - Completes the medical record documentation and ensures it is complete and reflects changes in patients’ condition and discrepancies between what is ordered and what is delivered. - Schedules/Participates in Integrated Plan of Care meetings and documents appropriately. - Participates in the vascular access monitoring program to prevent access failure; documents identified problems, notifies physician, and develops a corrective action plan. - Directly or indirectly reviews monthly the patient’s medication list (Home and In Center medication).
