Education: Bachelor of Science in Nursing (BSN). Experience: Minimum of 2 years in Neurology. Responsibilities: Follow patients who have experienced a stroke or seizure for 90 days post-discharge. Conduct follow-ups if patients are admitted to a Skilled Nursing Facility (SNF) or Long-Term Acute Care (LTAC). Schedule specialist or Primary Care Physician (PCP) follow-up appointments. Oversee medication management and coordinate home healthcare services. Assist in arranging clinic visits if concerns are identified. Preferred Qualifications: Prior experience in homecare or case management.
