Overview: Occupational Therapy works closely with PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety as well as enhance performance of ADLs. The Occupational Therapist collaborates with the Physical Therapist to efficiently operate WelbeHealth rehabilitation department and is a member of the WelbeHealth management team and facilitates collaborative relationships across WelbeHealth and with external stakeholders to promote the WelbeHealth values, team culture and mission. Conduct Occupational Therapy assessments to determine the Occupational Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans. Deliver and document Occupational Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans including but not limited to the use of devices, techniques and activities to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance and other factors affecting the level of independence in performing ADLs. Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP). Provide training and instructions to staff, participants and caregivers/families in areas of Occupational Therapy in which they may work with participants. Order, track and manage maintenance for equipment. Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants’ condition or functional status. Work with the other members of the IDT to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.). Participate in end-of-life care coordination and support. Manage direct reports including training, supervising and mentoring. Skills & Experience/Education: Minimum of two years Occupational Therapy experience in a clinical setting with a frail or elderly population. Bachelor’s Degree in Occupational Therapy from Accredited Program (AOTA and ACOTE standards). Initial Registration from National Board for Certification in Occupational Therapy. Licensed and/or eligible for Licensure as required in the State of California. BLS – American Heart Association. Job ID #15451342
