Job Description: Responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. San Jose/South Bay/Santa Clara County Requirements: - BLS (AHA) - 2 years of home health experience - CA PT Certification -EMR SYSTEM IS HCHB Shift: 5 x 8 Days 0800-1700 Weekend Requirements: Rotating Weekends-every 6-7 weeks
