Requirements: Current LPN License, BLS Graduation from an accredited school of practical nursing required. Submittal Packet Requirements: Cover Page, Resume, Proof of Required Certifications/Licensure (for RNs also include skills checklist and ensure that eNotify is uploaded) Important Note: Temp to Perm. Worker must be willing to convert after the contract is over. The worker must either live within 45 minutes of the facility or be relocating to the area. If your worker is not local, please indicate on the resume or cover page if your worker is in the process of relocating or plans to relocate permanently if offered. If this is not on the submittal packet, the worker will be rejected.
