Order Description Facility Name: The University of Vermont Health Network at Champlain Valley Physicians Hospital Total # of Facility Beds: 341 Unit Name: OR Unit Specialty: Surgical Services OR Total # of Unit Beds/Suites/Deliveries: 8 OR Main Campus and 4 ORs at ASC Teaching Facility: Y N Teaching Hospital Experience Required: Y N Trauma Level: 3 Prior Travel Experience Required: Y N Minimum # Years Clinical Experience Required: 2 Minimum # Years Clinical Experience in Specialty Required: 2 Certifications Required: NY lic, BLS Certifications Preferred: Yes, but not required. Note any certs Nurse Patient Ratio by shift: 1:1 Circulate/Scrub as applicable: Circulate required. Scrub experience preferred. Ancillary Assistance: CNA/ PCT: Y N #staff per shift: Unit clerk/Secretary: Y N #staff per shift: Other: OR Aides, Scrub Techs, Surgical Assists, RNs #staff per shift: 30 Advanced Practice on the unit: No # Days of Orientation: 2 Specialty Testing for Unit/Facility: U F Patient Care Modality: OR Dress Code Scrubs/color: Scrubs provided Shoes: closed toe, non-skid OR dress code Jewelry: None while scrubbed in. Earrings covered by hair covering. Hair/Beard: Cloth surgical caps must be covered by disposable cap. Beards must be covered. Nail Polish: Cannot be chipped and no fake nails Documentation System: EPIC Medication System: EPIC Floating Required: Y N To what units/facilities: ASC Describe the rotation: Rotation depends on cases scheduled and nurses experience Charge required: Y N Scheduling: Smartsquare Shift Hours: 0700-1930 Rotation Schedule: rotate days off with other staff that work 12s Self-Scheduling No Weekend Requirements: On call once every 3rd to 4th weekend Call Requirements: 1 night per week Holiday Requirements: If Holiday hasnt been assigned to a staff member will put them on call for that holiday. Only 1 holiday per year. Patient Population (Diagnoses): All surgeries except Cardiac, transplants, neuro, and gastric bypass Age Range: 6 months-100+ Special Equipment: (For example: IV Pumps: Type; Invasive Monitoring; Monitors: Type; ECMO; Ventilators: Type; ABG: Type) Respiratory available 24/7 for support Y N Anesthesia available 24/7 for support Y N Who manages ventilators: Respiratory Unit Specific Procedures and Protocols Start IVs, patient transports if central transport is backed up. Additional Unit Specific Information: Comments Assignment for 20-32 weeks TCertified OR Tech: Float and call required, weekend shifts every other (as needed) – not more than once a month if applicable; Max call 1x post shift/week, 1 weekend day per 4-week schedule period. Requirements: • BLS is required, CNOR preferred • Need cert through NBSTSA • Must have close to 2 years of experience • Prior travel preferred • Work history needs to be from similar size facility, as well as trauma II or higher • Solid knowledge base including, but not limited to ortho, robotics, general surgery, ENT, plastics, neuro Additional Information: • IVS to OFFER • Monday to Sunday • Bi-weekly Orientations on Monday • No block schedules for any reason. • This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit. • Please ensure all employment gaps over 30 days are explained • Covid and Flu declinations accepted- please have either proof or declinations in packet Disclaimers: • Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended. • Vehicles should be equipped with all season or snow tires during the months of November – April.
